Care and Support Planning project underway!

Having welcomed a brand-new team this summer, our charity’s life-changing Care and Support Planning project is now well under way!

As a national team, Community Integrated Care’s Assessment Intervention Practitioners are working from the top of Scotland to the tip of England, supporting our frontline colleagues to deliver the best lives possible for the people we support.

After a thorough induction, which included two weeks of in-person workshops delivered by internal teams and external partners, our 20 fantastic new colleagues are now visiting services across our regions.

The team is now working closely alongside our services to drive excellence in Care and Support Plans, ensuring that everyone is working together to achieve the same purpose – maximising independence and quality of life for the people we support. They’ll do this through coaching and mentoring our frontline colleagues to have meaningful conversations, with both the people they support and those who know them best.

These conversations are embedded within the guiding principles of Paradigm’s REACH standards, encouraging citizenship, widening people’s opportunities and ensuring that people are living a truly great life.

Excitingly, later this year the team will also be rolling out an immersive training experience for all frontline colleagues across our charity.

That’s why I’m delighted that with only ten weeks in our new team have already hit the ground running, thanks to so many of our frontline colleagues truly buying into this approach. I’m excited to see what the future holds for this special project and to witness the huge impact that it is going to have on so many of the people we support.”
