Video: Integration in Dementia Care, the St Luke’s, Runcorn story

On the 19th June 2015, Community Integrated Care held a special celebration event, which launched a £250,000 refurbishment of St Luke’s Care Home in Runcorn and celebrated our unique integrated partnership with Halton Borough Council and NHS Halton Clinical Commissioning Group.

This partnership has transformed St Luke’s in Runcorn from a traditional and struggling care home into a genuine centre of excellence for integrated dementia care. It sees NHS clinical specialists and local authority social care professionals work side by side with our caring colleagues every day, keeping people healthier and more independent for longer.

Together, we have jointly funded a £250,000 refurbishment of the home – creating a more dementia-friendly environment. Not only has this seen us improve communal and living spaces, but we have also created new themed activity areas, including a cinema, pub, indoor gardens and activities and reminiscence area.

This innovative integrated approach keeps local people who live with dementia enjoy happier, healthier and more independent lives. As a result, the home now supports people with the most complex and advanced dementia needs in Halton. It has also reduced demand on local health services and hospitals – helping the local NHS to use its resources more effectively.
